Patients with conditions that could be exacerbated by hypoxemia or hypercarbia (such as pulmonary hypertension or intracranial hypertension) or those at risk for significant complications as a result of sedation (such as with a full stomach) warrant caution and case-by-case consideration of premedication administration. Congenital heart disease (in the absence of pulmonary hypertension) is not a contraindication to midazolam administration perioperatively.
